Understanding Diabetes
Managing diabetes well starts with sound knowledge. This knowledge base explains the causes, symptoms, diagnosis, treatment and day-to-day management of diabetes in plain language, to help you understand the condition, build healthy habits and lower the risk of complications.
What is diabetes?
Diabetes mellitus is a common chronic condition. When the body does not produce enough insulin, or cannot use insulin effectively, glucose in the blood stays high. Poorly controlled blood glucose over time can damage blood vessels large and small, as well as the eyes, kidneys, nerves, heart and brain, raising the risk of complications. Understanding diabetes and managing blood glucose early is an important step towards staying well.
What is blood glucose?
Blood glucose is the sugar carried in the blood, and the body's main source of energy. Rice, noodles, bread, fruit and other carbohydrate foods are broken down into glucose during digestion, enter the bloodstream, and are delivered to organs and cells to fuel daily activity.
Normally the body keeps blood glucose within a suitable range, neither too high nor too low.
What does insulin do?
Insulin is a hormone made by the pancreas. Its main job is to help glucose move from the blood into cells, where it is used for energy or stored — keeping blood glucose stable.
When the body cannot make enough insulin, or cells respond poorly to it (insulin resistance), glucose cannot enter cells efficiently, blood glucose rises, and diabetes may develop.
Why does high blood glucose affect health?
Persistently high blood glucose disrupts normal body function and gradually damages blood vessels and nerves, increasing the risk of complications such as diabetic retinopathy, diabetic kidney disease, neuropathy, heart disease and stroke.
Because these changes develop slowly, detecting and controlling blood glucose early is important for protecting overall health.

A note from Angel of Diabetic
Diabetes often has no obvious symptoms in its early stages, and many people only discover raised blood glucose at a routine health check. Even without discomfort, high blood glucose can slowly damage the eyes, kidneys and cardiovascular system. If you are at higher risk — a family history, overweight or obesity, high blood pressure, abnormal blood lipids, or a previous finding of prediabetes — regular blood glucose testing is recommended.

What types of diabetes are there?
Diabetes is mainly grouped into type 1, type 2 and gestational diabetes, alongside some less common specific types. Each has different causes, typical age of onset and treatment, so understanding the differences helps with early recognition and appropriate care.
(Type 1 Diabetes)
Type 1 diabetes
In type 1 diabetes the immune system mistakenly attacks the insulin-producing beta cells of the pancreas, so the body produces almost no insulin. Lifelong insulin injections are required to control blood glucose.
Type 1 most often begins in children, adolescents or young adults, though it can occur at any age. Onset is usually rapid, with thirst, frequent urination, weight loss and fatigue. Without prompt treatment it can lead to diabetic ketoacidosis (DKA), a medical emergency requiring immediate care.
(Type 2 Diabetes)
Type 2 diabetes
Type 2 is the most common form, accounting for more than 90% of all cases. The body still produces insulin, but cells respond poorly to it (insulin resistance) and the pancreas gradually produces too little, so blood glucose stays high.
Type 2 is associated with family history, overweight or obesity, physical inactivity, an unhealthy diet and increasing age. With changing lifestyles, it is increasingly seen in younger people.
(Gestational Diabetes)
Gestational diabetes
Gestational diabetes refers to high blood glucose first identified during pregnancy, usually detected by a glucose tolerance test between 24 and 28 weeks.
For most women blood glucose returns to normal after delivery, but the risk of developing type 2 diabetes later is higher. Regular blood glucose monitoring and healthy habits after childbirth are therefore important.
Other specific types
Besides the three main categories, less common specific types exist — for example monogenic diabetes caused by genetic variants, diabetes arising from pancreatic or endocrine disease, or diabetes induced by medications such as steroids. These are relatively rare and usually require detailed assessment and treatment by a specialist.

What is prediabetes?
Prediabetes means blood glucose is higher than normal but has not reached the threshold for diagnosing diabetes. It is not diabetes, but it shows the body's ability to regulate glucose is already declining — an important warning sign.
Without changes to daily habits, some people with prediabetes go on to develop type 2 diabetes. Conversely, acting early — improving diet, increasing activity and managing weight — can bring blood glucose back to normal, which is why prediabetes is regarded as a key window for prevention.
Why does prediabetes occur?
Prediabetes is mainly linked to insulin resistance. As cells respond less well to insulin, the pancreas must produce more to keep glucose normal. Over time, when the pancreas can no longer keep up, blood glucose rises into the prediabetes range and may progress to type 2 diabetes.
Who is more likely to develop prediabetes?
Risk is higher for people who:
- Are aged 40 or above
- Are overweight or obese, particularly with central obesity (a large waistline)
- Are physically inactive
- Have a family history of diabetes
- Have high blood pressure or abnormal blood lipids
- Have had gestational diabetes or given birth to a large baby
- Have previously been told their blood glucose was high
If any of these apply to you, regular blood glucose testing is recommended.
How do I know if I have prediabetes?
Prediabetes usually causes no obvious symptoms and is most often found at a health check. Doctors generally assess it using:
6.9 mmol/L
11.0 mmol/L
- Glycated haemoglobin (HbA1c): 5.7% to 6.4%
- Fasting plasma glucose (FPG): 5.6 to 6.9 mmol/L
- Oral glucose tolerance test (OGTT): 2-hour glucose between 7.8 and 11.0 mmol/L
If results fall within these ranges, your doctor will advise and follow up based on your overall health.
Can prediabetes be reversed?
Yes. Research shows that many people with prediabetes can substantially lower their risk of progressing to type 2 diabetes by changing daily habits, and some return to normal blood glucose levels.
Useful starting points:
- Eat a balanced diet, reducing high-sugar and high-fat foods
- Do at least 150 minutes of moderate-intensity exercise each week
- Manage weight, especially abdominal fat
- Get enough sleep and reduce stress
- Test blood glucose regularly and follow up on your health
Acting early is usually more effective than starting treatment only after diabetes has been diagnosed.
Key points
- Prediabetes is not diabetes, but it means the risk of developing it has increased.
- Most people have no obvious symptoms; it is found through blood glucose testing.
- Improving daily habits can bring blood glucose back to normal.
- Regular monitoring helps prevent type 2 diabetes early.
A note from Angel of Diabetic
Prediabetes is not simply "nearly diabetic" — it is an important signal from the body. Because most people have no symptoms, it is usually found only at a health check. Acting at this stage can delay or even prevent type 2 diabetes, and also lowers the future risk of cardiovascular disease and other diabetes-related complications.

What are the symptoms of diabetes?
Diabetes may cause no obvious symptoms early on, so many people first discover raised blood glucose at a health check. As blood glucose rises, symptoms may begin to appear. Because each person's condition and glucose levels differ, the type and severity of symptoms vary.
Why do symptoms occur?
When blood glucose stays above normal, the body tries to remove the excess glucose through urine. This produces more urine and carries away a large amount of water, which can lead to dehydration — and the body responds with thirst.
At the same time, because glucose cannot enter cells efficiently to provide energy, cells remain "energy-starved" even though blood glucose is high. This causes fatigue and, in some cases, weight loss.
Common symptoms
Common symptoms of diabetes include:
- Persistent thirst — a dry mouth and the urge to drink even after drinking water.
- Frequent urination — particularly getting up several times at night.
- Fatigue — feeling low in energy despite adequate rest.
- Eating more but losing weight — as cells cannot use glucose, the body breaks down fat and muscle for energy.
- Blurred vision — fluctuating glucose can affect the lens of the eye, causing temporary blurring.
- Slow-healing wounds — high glucose affects circulation and immune function.
- Frequent infections — such as skin, urinary tract or candida infections.
- Numbness or tingling in the hands and feet — long-term high glucose can affect the nerves.
Does having no symptoms mean no diabetes?
Not necessarily.
Type 2 diabetes usually develops slowly. Many people have no obvious symptoms early on and may live with it for years without knowing. Even without discomfort, high blood glucose can continue to damage the eyes, kidneys, nerves and cardiovascular system — which is why some people already have complications at diagnosis.
If you are at higher risk — family history, overweight or obesity, high blood pressure, abnormal blood lipids, or a previous diagnosis of prediabetes — regular blood glucose testing is recommended even without symptoms.
Key points
- Diabetes may cause no obvious symptoms early on; many people are diagnosed at a health check.
- Common symptoms include thirst, frequent urination, fatigue, weight loss, blurred vision and slow-healing wounds.
- Long-term high blood glucose can damage the eyes, kidneys, nerves and cardiovascular system even without symptoms.
- People at higher risk should test regularly for early detection and management.
A note from Angel of Diabetic
Many people believe "no symptoms means no diabetes", but this is a common misunderstanding. Type 2 diabetes often causes no clear signs early on while high blood glucose is already doing harm. If you are at higher risk, do not wait for symptoms — a regular blood test is the key to early detection.

Who is more likely to develop diabetes?
Diabetes is not limited to older adults — anyone can develop it. However, genetics, lifestyle and existing health conditions make some people more likely to develop type 2 diabetes. Knowing these risk factors helps you make changes early and lower your risk.
A family history of diabetes
If a parent, sibling or other close relative has type 2 diabetes, your own risk is higher. Genetics cannot be changed, but a balanced diet, regular exercise and a healthy weight still help reduce risk.
Being overweight or obese
Excess weight, particularly abdominal fat (central obesity), increases insulin resistance, so the body must produce more insulin to keep glucose normal. Over time pancreatic function may decline, raising the risk of type 2 diabetes.
Physical inactivity
Exercise improves the body's sensitivity to insulin and helps control blood glucose. Long-term inactivity not only encourages weight gain but also raises the risk of type 2 diabetes.
Poor dietary habits
Frequently eating high-sugar, high-fat, high-calorie or processed foods, together with too few vegetables, fruits and whole grains, can lead to weight gain and metabolic problems that raise risk. Building balanced eating habits is an important step in prevention.
High blood pressure or abnormal blood lipids
High blood pressure, high cholesterol and high triglycerides often occur alongside type 2 diabetes. If you have these conditions, regular blood glucose monitoring is particularly important.
A history of gestational diabetes
Women who had gestational diabetes remain at higher risk of type 2 diabetes later, even if blood glucose returned to normal after delivery. Regular testing and healthy habits after childbirth are recommended.
Increasing age
Metabolism gradually slows with age, and reduced muscle mass and activity further increase the risk of type 2 diabetes. That said, type 2 diabetes is increasingly seen in younger people, so healthy habits matter at every age.
Key points
- A family history of diabetes and being overweight or obese raise the risk.
- Physical inactivity and an unbalanced diet increase the risk of type 2 diabetes.
- High blood pressure, abnormal blood lipids and a history of gestational diabetes are all risk factors.
- Healthy eating, regular exercise and a healthy weight help prevent type 2 diabetes.
A note from Angel of Diabetic
Having risk factors does not mean you will definitely develop diabetes, but it does mean paying closer attention to your health. If you are at higher risk, regular blood glucose testing and healthy daily habits give you the best chance of prevention.

How is diabetes diagnosed?
Diabetes may cause no obvious symptoms early on, so it cannot be judged by how you feel. If you are at higher risk, or have symptoms such as thirst, frequent urination, fatigue or weight loss, see a doctor early so that appropriate blood glucose tests can be arranged for timely diagnosis and treatment.
Which tests are used?
Your doctor will choose tests based on your situation. Common diagnostic methods include fasting plasma glucose (FPG), glycated haemoglobin (HbA1c) and the oral glucose tolerance test (OGTT).
Where appropriate, a random plasma glucose test may also be used for initial assessment, particularly for people with typical diabetes symptoms.
Fasting plasma glucose (FPG)
FPG is one of the most commonly used tests. You must not eat for at least 8 hours beforehand (plain water is allowed), after which blood is drawn to measure glucose.
Fasting plasma glucose results:
- 5.6–6.9 mmol/L:
- Below 5.6 mmol/L — normal range.
- 5.6–6.9 mmol/L — prediabetes.
- 7.0 mmol/L or above — meets the criteria for diabetes (usually confirmed by repeat or additional testing as advised by your doctor).
Glycated haemoglobin (HbA1c)
HbA1c reflects average blood glucose over the past 2–3 months and is not affected by a single day's eating, making it an important test for both diagnosis and monitoring.
In general:
- Below 5.7% — normal range.
- 5.7%–6.4% — prediabetes.
- 6.5% or above — meets the criteria for diabetes (subject to overall assessment by your doctor).
The next section explains HbA1c and how to interpret the results in more detail.
Oral glucose tolerance test (OGTT)
The OGTT is used mainly to diagnose prediabetes and gestational diabetes, and for people in whom other test results are inconclusive.
A fasting blood sample is taken first; you then drink a measured glucose solution, and blood is drawn again two hours later.
Two-hour glucose results:
- 7.8–11.0 mmol/L:
- 11.1 mmol/L
- Below 7.8 mmol/L — normal range.
- 7.8–11.0 mmol/L — prediabetes.
- 11.1 mmol/L or above — meets the criteria for diabetes.

Hong Kong generally uses mmol/L for blood glucose, while mg/dL is common in the United States. HbA1c is usually expressed as a percentage (%); some countries also use IFCC units (mmol/mol).
Is one blood test enough to confirm a diagnosis?
未必。
Not necessarily.
Without typical symptoms, doctors generally arrange a repeat test or use a second method to confirm the result, so that a temporary factor does not affect the diagnosis.
If typical symptoms are present — thirst, frequent urination, weight loss — together with a random plasma glucose of 11.1 mmol/L or above, a doctor may make the diagnosis on clinical grounds.
Key points
- Diabetes is diagnosed by blood glucose testing, not by symptoms alone.
- Common tests are fasting plasma glucose (FPG), HbA1c and the oral glucose tolerance test (OGTT).
- Each test has its own use; your doctor will choose what suits your situation.
- Without typical symptoms, results are usually confirmed by repeat or additional testing.
A note from Angel of Diabetic
The earlier diabetes is found and blood glucose brought under control, the greater the chance of reducing complications affecting the eyes, kidneys, nerves and cardiovascular system. If you are at higher risk, regular testing is advisable even without symptoms.

What is HbA1c?
HbA1c (glycated haemoglobin) is an important measure for diagnosing and monitoring diabetes, reflecting average blood glucose over the past 2–3 months. Unlike a single glucose reading, HbA1c is not greatly affected by one meal or one day, so it gives a fuller picture of long-term control.
Whether diagnosing diabetes, assessing treatment or following up over time, HbA1c is a test doctors rely on.
Why does HbA1c reflect the past 2–3 months?
Red blood cells live about 120 days on average. When blood glucose rises, some glucose binds to the haemoglobin inside red blood cells, forming glycated haemoglobin (HbA1c).
If glucose is high over a long period, more HbA1c forms; if control is good, HbA1c is lower. HbA1c therefore reflects average glucose over roughly 2–3 months rather than a single day.
What do the numbers mean?
|---|---|
| 5.7–6.4%(39–46 mmol/mol) |
| HbA1c | Interpretation |
|---|---|
| Below 5.7% (<39 mmol/mol) | Normal |
| 5.7–6.4% (39–46 mmol/mol) | Prediabetes |
| 6.5% or above (≥48 mmol/mol) | Meets the criteria for diabetes (subject to overall assessment by a doctor) |
For people already diagnosed with diabetes, HbA1c is also used to assess control. Treatment targets differ from person to person; your doctor will set a suitable HbA1c target based on age, general health, how long you have had diabetes and other conditions.
Is a lower HbA1c always better?
未必。
Not necessarily.
For most non-pregnant adults, an HbA1c below 7% (53 mmol/mol) is a common treatment target — but lower is not automatically better.
Lowering glucose too quickly or aiming too tightly can increase the risk of hypoglycaemia, especially in older adults and those using insulin or certain glucose-lowering medicines. Your HbA1c target should be set by your doctor for your situation, not by comparing numbers with others.
What can affect the accuracy of HbA1c?
Although HbA1c is an important measure, it is not suitable for everyone.
Results may be affected in people with severe anaemia, certain haemoglobin disorders, recent significant blood loss, a recent transfusion, or advanced kidney disease. In such cases doctors may combine other blood glucose tests for an overall assessment.
How does HbA1c relate to average glucose?
HbA1c reflects average glucose over roughly 2–3 months, not a single day or a single test. The table below shows HbA1c alongside estimated average glucose (eAG) to help you interpret your result. These are averages derived from large studies, and individual results may differ.
|---|---|
| 6.0%(42 mmol/mol) | 約 7.0 mmol/L(126 mg/dL) |
| 7.0%(53 mmol/mol) | 約 8.6 mmol/L(154 mg/dL) |
| 8.0%(64 mmol/mol) | 約 10.2 mmol/L(183 mg/dL) |
| 9.0%(75 mmol/mol) | 約 11.8 mmol/L(212 mg/dL) |
| 10.0%(86 mmol/mol) | 約 13.4 mmol/L(240 mg/dL) |
| HbA1c | Estimated average glucose (eAG) |
|---|---|
| 6.0% (42 mmol/mol) | About 7.0 mmol/L (126 mg/dL) |
| 7.0% (53 mmol/mol) | About 8.6 mmol/L (154 mg/dL) |
| 8.0% (64 mmol/mol) | About 10.2 mmol/L (183 mg/dL) |
| 9.0% (75 mmol/mol) | About 11.8 mmol/L (212 mg/dL) |
| 10.0% (86 mmol/mol) | About 13.4 mmol/L (240 mg/dL) |
eAG(Estimated Average Glucose,估算平均血糖)是根據 HbA1c 推算出的平均血糖值,只供參考,不能取代日常血糖監測或醫生的專業評估。
eAG (estimated average glucose) is calculated from HbA1c. It is for reference only and does not replace day-to-day glucose monitoring or professional assessment.
Understanding your own HbA1c

How can blood glucose be controlled?
Diabetes cannot yet be cured, but with active management most people can control blood glucose well and reduce the risk of complications affecting the eyes, kidneys, nerves and cardiovascular system.
Control does not rely on medication alone — it combines healthy eating, regular exercise, taking medicines as prescribed, and good daily habits. Treatment plans and glucose targets differ from person to person, so follow the advice of your doctor and care team.
Healthy eating is the foundation
A balanced diet helps stabilise blood glucose and maintain a healthy weight.
Choose more high-fibre foods such as whole grains, vegetables, pulses and fruit, with moderate amounts of lean meat, fish, eggs and low-fat dairy. Reduce sugary drinks, desserts, processed foods and high-fat foods, which make control harder.
People with diabetes are not forbidden sweet foods — what matters is the type, the portion and how it fits into the whole day's eating.
Regular exercise improves control
Exercise increases the body's sensitivity to insulin and helps glucose enter cells, lowering blood glucose.
At least 150 minutes of moderate-intensity aerobic activity each week — such as brisk walking, swimming or cycling — is generally recommended, along with some resistance training. Before starting a new exercise programme, consult your doctor if you have heart disease, diabetes complications or other health problems.
Take medicines as prescribed
Many people need oral glucose-lowering medicines or insulin to control blood glucose.
Even if your readings improve, do not stop, reduce or change your medication on your own — blood glucose may rise again and increase the risk of complications. If you are worried about side effects or experience hypoglycaemia, speak to your doctor or pharmacist promptly.
Monitor blood glucose and HbA1c regularly
Day-to-day testing shows how glucose changes — fasting, after meals, around exercise, or when hypoglycaemia is suspected — making it easier to adjust food, activity or medication.
HbA1c, meanwhile, reflects overall control across roughly 2–3 months and is an important measure of how well treatment is working.
The two serve different purposes; used together they give a fuller picture.
Build healthy daily habits
Beyond diet, exercise and medication, healthy habits also help stabilise blood glucose: maintaining a healthy weight, sleeping well, quitting smoking, avoiding excessive alcohol and learning to manage stress are all important parts of diabetes care.
Key points

- Take medicines as prescribed; do not stop or reduce them on your own.
- Daily monitoring and HbA1c serve different purposes and work best together.
- Sustained control reduces the risk of diabetes complications.
A note from Angel of Diabetic
Controlling blood glucose is a long-term process. Do not be discouraged by an occasional high or low reading. Keeping up healthy habits, taking medicines on time, monitoring regularly and attending follow-up appointments matters more than pushing glucose very low in a short time.

What are the common complications of diabetes?
When diabetes is poorly controlled over a long period, persistently high blood glucose damages blood vessels and nerves throughout the body, raising the risk of a range of complications. These do not appear immediately, and many people have no obvious symptoms early on — which is why regular checks and good glucose control matter.
That said, with early diagnosis, active glucose control and attention to blood pressure, blood lipids and healthy habits, many complications can be delayed or even prevented.
Microvascular complications
Long-term high blood glucose damages small blood vessels, mainly affecting the eyes, kidneys and nerves.
Diabetic retinopathy
High glucose can damage the blood vessels of the retina. There may be no symptoms at first, but as it progresses it can cause blurred vision, floaters and, in severe cases, blindness. Even with normal vision, people with diabetes should have regular retinal examinations.
Diabetic kidney disease
Diabetes is a leading cause of chronic kidney disease. There are usually no obvious symptoms early on; doctors assess kidney function through urine and blood tests. Without early control it may progress to kidney failure.
Diabetic neuropathy
High glucose can damage peripheral nerves, causing numbness, tingling, burning sensations or reduced feeling in the hands and feet. If a foot injury goes unnoticed, the risk of infection and foot ulcers rises.
Macrovascular complications
Beyond small vessels, diabetes also increases the risk of atherosclerosis, which can lead to heart and cerebrovascular disease.
Common macrovascular complications include:
- Coronary heart disease and myocardial infarction (heart attack)
- Stroke
- Peripheral arterial disease (affecting circulation in the legs)
Risk increases further with high blood pressure, high cholesterol, smoking or obesity.
Foot problems should not be overlooked
Because of nerve damage and poorer circulation, people with diabetes are more prone to foot wounds, infections and ulcers. Untreated, these can become severe enough to require amputation.
Check both feet daily, keep them clean and dry, and wear well-fitting shoes and socks. If a wound is slow to heal, seek medical advice promptly.
Good control lowers the risk
Complications are a real concern, but they do not affect everyone with diabetes.
Research shows that good blood glucose control — alongside managing blood pressure and blood lipids, maintaining a healthy weight, not smoking and attending regular checks — helps reduce risk and protects the eyes, kidneys, heart and other vital organs.

Which regular check-ups are needed?
Alongside taking medicines and monitoring blood glucose, people with diabetes should attend regular examinations to assess control and detect complications of the eyes, kidneys, heart and other organs early.
Many complications cause no obvious symptoms at first, so attend the checks your doctor recommends even if you feel well.
HbA1c(糖化血紅蛋白)
HbA1c (glycated haemoglobin)
HbA1c reflects average blood glucose over roughly 2–3 months and is a key measure of long-term control.
Testing every 3 to 6 months is generally recommended; the actual frequency depends on your control and treatment plan.
Blood pressure
High blood pressure increases the risk of heart disease, stroke and kidney disease, so blood pressure should be measured at every follow-up visit and kept within the target range advised by your doctor.
Blood lipids
People with diabetes are more prone to abnormal blood lipids, which raise cardiovascular risk.
A blood test at least once a year is generally recommended to check cholesterol and triglycerides; your doctor will decide whether treatment needs adjusting.
Kidney function
Diabetic kidney disease usually causes no symptoms early on, so an annual assessment is recommended, including:
- Urine test (urine albumin-to-creatinine ratio, UACR)
- Blood test (kidney function and estimated glomerular filtration rate, eGFR)
Regular testing allows kidney damage to be found and treated early.
Dilated eye examination
Diabetes can cause retinopathy, which may produce no vision problems at first.
An annual dilated retinal examination is generally recommended to detect retinopathy early and reduce the risk of vision loss.
Foot examination
Your doctor will check sensation, circulation and skin condition in both feet to assess the risk of neuropathy and foot ulcers.
Besides regular examinations, check your feet daily for wounds, blisters, redness, swelling or other changes, and seek advice promptly if you find any.
Dental check-ups
People with diabetes are more prone to gum disease, which in turn can affect blood glucose control.
Regular dental examinations and cleaning are recommended, along with good oral hygiene.

A note from Angel of Diabetic
The purpose of regular check-ups is not only to find disease, but to detect complications before symptoms appear so they can be treated early. At each visit, it is worth asking your care team whether you have completed all the recommended diabetes checks.
